What you share in therapy is protected to the full extent of the law and social work ethics.
Your information will never be shared unless you present an immediate lethal harm to yourself or others, or in cases of the abuse of a child, elder, or disabled person. In such cases, I always attempt to make the client aware that a report is being made. If you want me to share information for a medical, legal, professional, or personal reason, you will sign a release of information form.
I use a HIPAA-compliant platform to store your information and therapy record, and to provide telehealth. I only email clients about scheduling issues. I do not send texts to clients. You may periodically need to reach out to me by phone for support. These encounters, and any clinical information you might send to me via email or text, will be documented in your therapy record.
My fee is 150 dollars for a 50 minute session. I will bill you at the end of each month.
I accept credit cards, checks, cash, and health savings accounts. You may store your credit card number in my protected patient portal and enroll in autopay at your convenience. If you are not enrolled in autopay, I respectfully request that you pay your bill within one week of receipt.
I do not work with insurance at this time. Upon request, I can provide you with a receipt of treatment that you can use to file for any out of network benefits you may have.
When determining your out of network coverage, you need to ask your insurance company:
1) Does my plan include out of network mental health benefits and do I need preauthorization to access those benefits?
2) What is the out of network deductible under my plan and has part or all of it been satisfied?
3) What percentage of the cost of treatment will be my responsibility once my out of network deductible has been met?
4) Is there a limit to the number of sessions my plan will cover?
You must cancel your appointment 24 hours prior to your appointment. I charge the full fee for any missed or late canceled appointments.
As of January 1, 2022, the No Surprises Act requires medical providers to give patients seeking care without insurance a Good Faith Estimate (GFE) of the cost of treatment at the point of seeking care.
Guided by the National Social Workers’ Code of Ethics, I reserve adequate time to assess and diagnose a client and plan for treatment. Therefore, I will provide you with a diagnosis and GFE at some point during your first three sessions. The GFE will estimate the cost of 6 months of therapy and will be reissued every 6 months.
